Our team of Social Workers are also known as Care Managers. Our aims are:
– To provide advice, information and support.
– To enable you to help yourself during times of crisis and change.
– To promote choice, control and independence.
– To protect vulnerable people.
To be able to receive services from the Community Team for People with Learning Disabilities (CTPLD) you must meet our eligibility criteria. You may also need to refer to information about the National Eligibility Determination Assessment (NED), Mental Capacity and The Care Act 2014.
Please be aware that you may not be eligible to receive services from our team. If this is the case you will be referred to North Somerset Council’s Adult Care Team.
If you are referred to the Community Team for People with Learning Disabilities (CTPLD) you will receive an assessment to find out what your needs are.
If your level of need means you are able to receive services from us you will be allocated to a Social Worker.
Your Social Worker will work with you to make sure that you are receiving the right care and support.
Your support plan will be reviewed regularly, either by a Social Worker or Community Reviewing Officer.
We work with other professionals to support you to receive the services you require.
We will make a referral to the Finance Team at North Somerset Council who will work out if you can afford to pay any money towards your care and support.
If you have health needs you may also be referred to another member of our team who can talk to you about the health choices available.
